10/06/15 15:04 initial exam microchip scan negative approx 8 week old male intact kitten all deciduous teeth are in ulcerated nasal philtrum/planum severe conjunctivitis OU – OS: eye appears to have ruptured and healed over, OD: globe appears WNL (hyperemic 3rd eyelids are up bilaterally obscuring the surface of the cornea), MP ocular discharge OU ears mites AU, very dirty ears (cleaned during exam) flea dirt seen (applied frontline spray) sneezing, mild nasal discharge pale pink mm’s underweight (BCS 2/9) no lingual ulcers eating well during exam PLAN: FVRCP vaccine 1/2 tab capstar given during exam, applied frontline spray for fleas move to URI start doxy 0.15 ml PO SID x 10 days — gave first dose during exam start erythromycin OU BID x 10 days start LRS 15 ml SQ SID x 3 days start nutrical 1.5 ml BID x 5 days start nebulization BID x 5 days clean ears SID x 3 days tresaderm AU BID x 14 days
