Medical Summary INITIAL EXAM microchip scan negative approx 2 year old male intact DSH cat behavior: vocal, friendly BAR, hydrated, mm’s pink and moist male, intact, 2 scrotal testes slightly underweight (BCS 3/9) heart and lungs WNL on auscultation amb x 4, no lameness noted fleas (live fleas and flea dirt) and ear mites superficial abrasions on pinnae (suspect self inflicted trauma due to ear mites) abdomen soft and non-painful, no masses palpated adult dentition with mild staining no URI signs PLAN: gave FVRCP vaccine SQ over right front leg gave rabies vaccine SQ over right hind leg applied activyl topically gave 1.2 ml pyrantel PO gave 0.2 ml ivermectin PO for ear mites sign up for ear cleaning and start tresaderm AU BID x 10 days sign up for convenia injection (0.6 ml SQ once) for abrasions sign up for chlorpheniramine to reduce itchiness sign up for capstar PO once prognosis: good
