*** SAFE 05/19/15 *** WONDERFUL WHISKERS IS A FIVE YEAR OLD STRAY IN NEED OF A FRIEND…..WHISKERS has been out on his own for a bit and it shows….he may be blind in one eye, may have a heart murmur and has suffered from ear mites. Despite these difficulties, WHISKERS has an AVERAGE rating. PLEASE HELP THIS QUIET, MELLOW GUY FIND A “FUREVER” HOME TONIGHT WHERE HE CAN FIND THE LOVE AND CARE HE HASN’T HAD…..OFFER TO FOSTER OR ADOPT THIS TERRIFIC TUXIE TONITE!!

I am a male black and white domestic sh mix. The shelter thinks I am about 5 YEARS old.
I came in the shelter as a STRAY on 05/04/2015 from NY 10308, owner surrender reason stated was STRAY.

05/11/2015 Exam Type RE-EXAM – Medical Rating is 3 C – MAJOR CONDITIONS , Behavior Rating is AVERAGE, Weight 10.7 LBS.
Hx: Recheck ears after ear mite tx (ivermectin and tresaderm/otibiotic). Pt has hx of cloudy eyes, heart murmur and scratches behind ears. Currently pt has a good app for dry food. S: Quiet, prefers to be sitting in cage rather than handled. Tolerated first half of exam, but growling and swatted by the end. Pt currently wearing E-collar. O: QAR-H, BCS 6/9, MMs pink and moist, CRT <2 sec EENT: Fur on face is greasy and unkempt. OS has spherical crystalline object in the anterior chamber. OD WNL. AU greasy yellow medication on pinnae; otoscopic exam not performed today. Fresh and healing scratches just caudal to the pinnae. Pinnal-pedal reflex present. Pt shakes head regularly. H/L: Grade 4/6 systolic heart murmur, PMI left side. NSR. Eupnic, quiet lung sounds. Abd: Slightly large, no pain on palpation, no masses palpated M/S/I: Amb x4. Skin of dorsal neck is thickened and alopecic. UG: Male intact, testicles soft and symmetrical. A: 1. OS crystalline object – R/O cataract and anterior lens luxation vs. neoplasia vs. other 2. Heart murmur 3. Ear mites – treated, ears still itchy, did not do otoscopic exam today so unclear if problem is resolved 4. Self-trauma to dorsal neck and head Short-term prognosis: Good to poor. Anterior lens luxation may be primary or secondary to another issue. Often glaucoma is a sequela. Usual tx is lens removal. Pt needs further diagnostics and ophthalmologic exam to determine true prognosis. P: 1. Move out of Adoptions 2. Consider placing through NH vs, EHR 1088

Notes: Whiskers was brought to the care center as a stray so we don’t have any behavioral history or tendencies previous to was seeing during the evaluation. At the time of the assessment he was displaying friendly behavior, comes to the front of the kennel, interacts with the Assessor, is easy to handle and tolerates all petting. Whiskers is displaying behavior appropriate for a beginner cat owner. Reaction to assessor: does not come to front but looks at you, is calm, relaxed and friendly (1) When spoken quietly: comes to the front of cage (1) Reaction to door cage opening: remains soft and relaxed (1) Reaction to touch: allows petting and attention (1) Try picking cat up: tolerant of this type of handling Placement Determination: Average
