S/O: Appearance: BARH, eating very well when offered food Integument: Appropriate hair coat, normal nails EENT: OU clear with raised 3rd eyelid, AU clean, No nasal d/c Oral cavity: MM pink, CRT <2sec, Healthy dentition PLN: WNL H/L: No murmur or arrhythmias, Lungs clear HR 186 ABD: Soft non painful, U/G: MI M/S: Amb x 4, supernumerary digits on both front paws BCS: 4/9 Neuro: ataxia, crouched stance when walkiing, mild intention tremor when eating, both 3rd eyelids raised A: ataxia with bilateral raised 3rd eyelid r/o cerebellar hypoplasia, toxin, infectious disease, trauma P: combo and parvo test neg, monitor for worsening of symptoms
