I came in the shelter as a OWNER SUR on 05/04/2015 from NY 11203, owner surrender reason stated was OWNER SICK.

S/O: Vet check for Geriatric exam. patient is BAR on presentation. Cautious but allowed slow handling. Great appetite, normal BM, urine output. Limited exam allowed due to temperament. mm = pink, moist hydration = adequate Integ = clean, kempt coat EENT = clear OU, did not examine ears. NND, Only canine teeth able to be examined which appear to be clean. H/L = nma, nsr, clear lung fields bilaterally Abdomen = snp CNS = approrpriate MS = BCS: 5-6/9 A: Apparently healthy Px: Good P: Continue to monitor while at BACC

05/04/15 4:21pm BASIC INFORMATION: Teddy is a 11 year old black and white neutered cat, with no health problems. Owner states that she bought Teddy from a pet store. The client is surrendering Teddy because she is sick and can no longer care for him. Social Life & Personality: Around children Teddy does not like to interact with kids he likes to stay away from them, she prefers that he goes to a home with no children, he lives in a home with no children; in a home he is very affecionate and loves to cuddle. Teddy is relaxed, and calm. Teddy has lived in a home with dogs, and he gets along with the dog they eat together and enjoys each others company. Teddy enjoys eating wet food, his favorite food is friskies. He loves to sleep in his bed, and when someone is home he loves to follow them around. Behavior Issues: Teddy has never bitten or scratched anyone in the time he was with his owner. Teddy is unbothered by being held, bathed, but he does not like to be put in a carrier. For a New family to Know: Teddy uses clumping litter in his hooded litter box. He enjoys playing with string, toy mice and chasing his toys. Teddy has not had a scratching post since he was a kitten.

Owner Profile: Teddy was bought into the care center by his previous owner because he is ill and can no longer care for him. Teddy did not lived with children but the previou owner stated he not does like to interact with kids he likes to stay away from them. Teddy is relaxed, and calm. Teddy has lived in a home with dogs, and he gets along with the dog they eat together and enjoys each others company. Notes: Teddy seems to not be thriving in this environment and at the time of the assessment he was displaying behaviors that preclude placement in the adoptions room and may require further investigation before placement in a home. He is fearful and currently does not tolerate much petting or handling. The behavior department feels that placement with a New Hope Partner is the best option at this time. Reaction to assessor: does not come to front but remains neutral (2) When spoken quietly: does not come to front but remains neutral (2) Reaction to door cage opening: becomes stiff, alert (3) Reaction to touch: growls, hisses does not allow much petting (5) Try picking cat up: did not attempt to pick the cat up, fractious behavior Placement Determination: NewHope
