**SAFE 09/26/15** SECOND CHANCE for this BEGINNER kitty—please make tonight count! SALLY IS SPECIAL….Only a year old she came into the ACC limping and undernourished….yet she has a BEGINNER rating!! Life has not been kind to SALLY but she endures and makes the best of it and now the ACC wants to end all that…..SALLY IS EATING WELL AND LOOKING FOR ATTENTION, ALL SIGNS OF A CAT WHO NEEDS TO BE SAVED!! She will need some follow up vet care to determine the cause of her atrophy and limping but that is certainly no reason for her to die!! 09/24/2015 Exam Type RE-EXAM – Medical Rating is 4 NC – SEVERE CONDITIONS NOT CONTAGIOUS, Behavior Rating is BEGINNER, Weight 5.2 LBS.
S/O: Sally is easily handled but growling throughout exam. She makes no attempts to swat or bite Appearance: QARH Integument: intact hair coat, normal nails EENT: OU clear, AU clean, No nasal d/c Oral cavity: MM pink, CRT <2sec, fractured maxillary canines PLN: WNL H/L: No murmur or arrhythmias, Lungs clear ABD: Soft non painful, large bladder, no palpable abnormalities U/G: female, normal external genitalia M/S: Amb x 4 with intermittent lameness on the right hind limb, RHL is severely muscle wasted and there is soft mass (~4cm x2cm) palpable above the proximal aspect of the femur BCS: 3/9 Neuro: NSF A: Hind limb muscle wasting/lameness/mass- r/o neoplasia vs. other P: Continue to monitor and seek treatment, as muscle wasting implies chronicity, no need to renew buprenex at this time.

09/19/15 19:15 Sally is a gray female tabby cat who was brought into ACC today as a stray. Sally was emmaciated and also was walking with a limp and she will undergo a full physical health exam. She was nervous but well behaved throughout the Intake process. She allowed herself to be collared and held for a brief time. She also allowed herself to be transferred into a new carrier with minimal effort. Sally became nervous when the carrier was being closed but did not display any negative behavior.

Notes: Sally was brought to the care center as a stray so we don’t have any behavioral history or tendencies previous to what we are seeing during the evaluation. At the time of the assessment she was displaying friendly behavior, comes to the front of the kennel, interacts with the Assessor, solicits attention, is easy to handle and tolerates all petting. Sally is displaying behavior appropriate for a beginner cat owner. Reaction to assessor: Sally was resting at the front of her cage When spoken quietly: Sally got up and started seeking attention Reaction to door cage opening: remains soft and relaxed Reaction to touch: solicits petting and attention Try picking cat up:Sally is tolerant of this type of handling Placement Determination: Beginner
