Microchip: negative Sex: neutered male Age: appx 9y Mentation: BAR, slight ataxia when walking (seems like left hind leg has most difficulty, no pain on palp) Eyes: sunken (dehydrated) otherwise clear Ears: waxy Nose: d/c, possible URI, some harsh sounds Teeth: calculus and wear, moderate to heavy; pale mm with slight jaundiced appearance If abnormal BCS: skinny with bony prominences but pendulous belly (feels like muscle wasting) Skin: prolonged turgor, dehydrated, heavy dander Hair Coat: poor, unkempt, some mats, urine odor Declawed: no Any injuries: no Behavior: easy to handle but does not enjoy being lifted, cautious Medication: N/A, preventatives given Moved to medical, eats and drinks readily
