I am a male black domestic sh mix. The shelter thinks I am about 2 YEARS old.
I came in the shelter as a STRAY on 04/13/2015 from NY 11379, owner surrender reason stated was STRAY.

04/14/2015 Exam Type INITIAL – Medical Rating is 3 C – MAJOR CONDITIONS , Behavior Rating is NONE, Weight 7.6 LBS.
intact male scan negative occular discharge present along with yellow nasal discharge 1049 confirmed URI and stated to start on doxycycline 50mg/mL PO SID for 10 days slight yellow staining of teeth present very scared and nervous, hissed once, didn’t strike, but will flee 0081 had to tranq for initial exam to be completed – gave .1 Telazol slightly dirty ears – cleaned ears – ear mite negative clean coat – flea negative QAR NOSF

Notes: RC Car was brought to the shelter as a stray so we don’t have any behavioral history previous to what we are seeing during the evaluation. He seems to not be thriving in this environment and at the time of the assessment he was displaying behaviors that preclude placement in the adoptions room and may require further investigation before placement in a home. He is extremely fearful and currently does not tolerate petting or handling. The behavior department feels that placement with a New Hope Partner is the best option at this time. Reaction to assessor: was hiding when I approach cage (3) When spoken quietly: continues to hide (3) Reaction to door cage opening: becomes stiff, alert (3) Reaction to touch: hisses, strikes, Try picking cat up: did not attempt to pick the cat up, fractious behavior Placement Determination: NewHope
