S/O: BAR, no ocular or nasal discharge. Tachypnea, and dysonea, greater effort during inspiration. Auscultation revealed HR >200pbm, gallop rhythm, decreased BV sounds. Suspect pleural space disease
Thoracic radiographs revealed pericardial diaphragmatic hernia.
A: Diaphragmatic hernia
P: Placed in supplemental oxygen cage, will notify NH, deadline of 5pm tonight due to severity of respiratory and cardiovascular compromise.
