Mrs Botts A1056496 – 9 week female brown tabby

S: calm and easily handled, rejected from surgery from Glendale due to heart murmur

O:

Appearance: BARH

Integument: Appropriate hair coat, normal nails

EENT: OU clear, AU clean, No nasal d/c

Oral cavity: MM pink, CRT <2sec. healthy juvenile dentition

PLN: WNL

H/L: significant “washing machine” murumur, Grade VI/VI (audible with stethescope off chest, palpable thrill), Lungs clear

ABD: Soft non painful, no palpable masses or organomegaly. mild abdominal distention

U/G: normal external genitalia

M/S: Amb x 4, no appreciable lameness

Neuro: NSF

A: Grade VI/VI- r/o congenital cardiac abnormalities (PDA, septal defects, other)

P: Very poor prognosis with VI/VI murmur in young kitten, consider EHR

Prognosis: Poor
