Moderate tartar and gingivitis, both maxillary canines are fractured.. PLNs: Not enlarged. H/L: NSR, NMA. Eupnic, quiet lung sounds. Abd: Soft, no pain on palpation, no masses palpated M/S/I: Polydactyly in all four feet. Amb x4. No lesions noted. UG: Male neutered A: Moderate dental disease and fractured maxillary canines. Polydactyl. Short-term prognosis: Excellent P: Long-term veterinary care should include dental cleaning and extractions 1088
