Medical Summary QARH scan negative mild tartar neutered reported cat has 2 broken bones in the right front leg. He has a broken jaw. Fell out of a 4 story window on July 30, 2015. Was being treated at ASPCA hospital. No paperwork provided. Luckie has been constipated for 2 weeks as well. Last seen at ASPCA hospital on 8/21/15 to change cast on leg because it was wet. came with cast on rt front limb and e collar on did not like to walk AMB x 3 spoke to Dr Dolbashian and explained gave 0.25cc Bupren 0.5mg/ml NOSF
