11/19/15 18:39 INITIAL EXAM microchip scan positive approx 5 year old female (presumed) intact DSH cat BAR, mild dehydration heart and lungs WNL – no murmur ausculted, lungs clear bilaterally no fleas or flea dirt seen ears clean AU eyes clear OU, no discharge nose WNL, no discharge, no congestion or URI signs no alopecia, haircoat covered in urine on hind end, fecal material under the tail mentally appropriate tail is limp, defecating during exam (appears incontinent, feces in rectum easily expressed) large full bladder able to be expressed ambulatory x 4, no lameness noted, underweight (BCS 3/9), no signs of trauma lymph nodes WNL clean adult dentition, oral cavity WNL prognosis: poor dx: urinary and fecal incontinence likely seconary to tail injury PLAN: gave rabies vaccine SQ over right hind leg gave FVRCP vaccine SQ over right front leg gave 0.8 ml pyrantel PO applied activyl topically start buprenorphine 0.1 BID x 5 days for pain associated with tail injury needs butt bath and apply SSD to areas with urine/fecal scald recommend NH placement vs. EHR if no placement found at end of stray hold
