Jimmy A1052328 – 11 week male gray tabby

S/O: Very dull, face down in puddle of urine, minimally responsive

INTEGUMEN: Intact hair coat, prolonged skin turgor

EENT: mild ocular discharge OU, no active nasal discharge, increased upper respiratory noise

Oral cavity: MM pink, CRT <2sec, Healthy juvenile dentition

PLN: WNL

H/L: No murmur or arrhythmias, heart rate 120 beats/minute, crackles in the right lung field, markedly increased respiratory effort

ABD: Soft non painful, no obvious masses or organomegaly

U/G: normal external genitalia

M/S: sternally to laterally recumbent, will fall over to the side when picked up

BCS: 4/9, mild generalized muscle wasting

Neuro: NSF

A:

1. respiratory crackles/increased RR/RE- r/o pneumonia vs. ARDS vs. non-cardiogenic pulmonary edema vs. other

2. Bradycardia-r/o secondary to cardiovascaular shock secondary to dehydration and infectious disease

3. 7-10% dehydrated-r/o secondary to above

4. dull mentation-r/o secondary to above

P: Subcutaneous Fluids 50mL Q12

Baytril 5mg/.kg SQ Q24

Continue Doxy 10mg/kg liquid PO

EHR if RR/RE worsens, Plea NH for transfer to in hospital treatment for IVF, IV antibiotics, O2 support, 2pm deadline for treatment
