STRAY – EVALUATE, NO HOLD Reason NO TIME
Intake condition EXAM REQ Intake Date 12/16/2015, From NY 10456, DueOut Date 12/19/2015, I came in with Group/Litter #K15-042687

Medical Behavior Evaluation GREEN
Medical Summary Sex: Female, intact Scan: Negative Estimated age: 2 years Fleas: No flea dirt. Hx: Came in with 2 mo old kitten (very small for age) S: Active, walks around exam table, allows all handling O: BAR-H, BCS 4.5/9, MMs pink and moist EENT: No discharge OU, AU, nose. Clean teeth. PLNs: Not enlarged. H/L: NSR, NMA. Eupnic, quiet lung sounds. Abd: Severely distended, firm, no pain on palpation M/S/I: Amb x4. Small wound on dorsal tail about 2/3 of the way to the end – scab half-attached, wound appears to be linear, 1 cm long, and full-thickness. UG: Female, small thickenings at mammary glands A: 1. Tail wound 2. Distended abdomen – R/O large bladder vs. parturition-related disease vs. parasites vs. other 3. Lactating Short-term prognosis: Fair P: 1. Administered pyrantel 1 ml PO during exam 2. Monitor abdomen, consider rads 3. Monitor tail wound – recheck in 3 days 1088

Medical Behavior Evaluation GREEN
Medical Summary Sex: Male kitten Scan: Negative Estimated age: 9 weeks based on teeth (all deciduous teeth are in) Fleas: Some flea dirt. Hx: Came in with mom, who has been kept by finder for last 6 months (prior to this pt’s birth). Pt has tremor. S: Severe whole-body tremor, falls over sometimes, ravenous appetite, allows all handling O: BAR, BCS 2/9, MMs and pink food-covered EENT: No discharge OU, AU, nose. Clean deciduous teeth. PLNs: Not enlarged. H/L: NSR, NMA. Eupnic, quiet lung sounds. Abd: Soft, no pain on palpation, no masses palpated M/S/I: Falls over (likely neurological in origin), appears to be amb x4. Wet hind end and diarrhea staining on hind legs. UG: Male intact kitten Neuro: Severe whole-body tremor, occasionally falls over or drops head and it thumps on the table. Appears alert and oriented. A: 1. Severe tremor – R/O cerebellar hypoplasia secondary to in utero panleukopenia infection vs. other cause 2. Diarrhea 3. Severely underweight Short-term prognosis: Poor P: Sending to MACC for monitoring and further workup. Waived vaccines temporarily. 1088
