05/15/15 15:59 Basic Information: Destiny is a 7 year old Calico cat. Destiny was adopted from AC&C back in 2011. Owner stated Destiny maybe due to have litter box issues in the home and which is the rest owner had to surrender. Social Life and Personality: Destiny lived with one male owner for the past 4 years. Destiny loved with visitors came over especially children. Destiny is relaxed, affectionate and respectful towards everyone in the household. Destiny behavior is unknown towards cats and dogs. Owner stated Destiny activity level is to a medium and can be playful and quiet. Behavior Issues: Owner stated Destiny never bite or scratched anyone to her knowledge. She claws on furniture and meows only when hungry. Destiny was never given a bath or nail trim. Destiny likes to be picked up and held. Destiny has been having litter box issues for the past 2 months and only been using the bathroom on carpet. Owner never took Destiny to the vet for this reason but believes she has kidney failure. Destiny only uses Fresh Step for litter. For a New Family to Know: Destiny likes to look out the window and play with ball or strings. She likes to scratch on scratching post and play with catnip. She likes to sit on owner or rarely seek attention. She eats wet and dry food called Science diet mix with 9 lives twice a day. Intake Behavior: Upon intake Destiny was shy and quiet. She scanned positive for a microchip and allowed all handling.

Owner Profile: Destiny was bought into the care center because of litter box issues. Destiny lived with one male owner for the past 4 years. Destiny loved with visitors came over especially children. Destiny is relaxed, affectionate and respectful towards everyone in the household. Destiny behavior is unknown towards cats and dogs. Owner stated Destiny activity level is to a medium and can be playful and quiet. Destiny has been having litter box issues for the past 2 months and only been using the bathroom on carpet. Owner never took Destiny to the vet for that reason. Notes: At the time of the assessment she was displaying friendly behavior, comes to the front of the kennel, interacts with the Assessor, solicits attention, is easy to handle and tolerates all petting. Destiny is displaying behavior appropriate for a beginner cat owner. Reaction to assessor: does not come to front but looks at you, is calm, relaxed and friendly (1) When spoken quietly: comes to the front of cage, soliciting, rubbing against front (1) Reaction to door cage opening: remains soft and relaxed (1) Reaction to touch: solicits petting and attention (1) Try picking cat up: tolerant of this type of handling Placement Determination: Beginner

Scanned positive for chip #4A1B50262C BARH – growls, hisses, allows short period of minimal handling Spayed ~7 years (see entry 11/04/08) EENM clear NOSF Activyl applied Owner claims cat may have kidney failure because it is urinating all over the house. Cat shows no discomfort on abdominal palpation (loose, pendulous abdomen and weight distribution typical for spayed female) – a bit tense around the bladder; bladder itself is soft.
