INTIAL EXAM:
S/O: found outside unable to walk, MN about 2yo
Appearance: BARH, very sweet, laterally recumbent
Integument: soaked in urin, normal nails
EENT: OU clear, AU copious discharge, pruritic, No nasal d/c
Oral cavity: MM pink, CRT <2sec, Healthy dentition
PLN: WNL
H/L: No murmur or arrhythmias, Lungs clear
ABD: severely firm, distended bladder
U/G: MI, very erythematous penis and peripenile area with hair clumps, able to urinate small amounts when bladder pressure applied
M/S: non amb
BCS: 7/9
Neuro: mentation appropriate

A: FLUTD with mild azotemia
P: unblocked with red rubber catheter, CBC chem performed, 0.5 cc dexamethasone SC, 0.5 cc cerenia SC, 0.5 cc penG SC, 200 cc LRS SC, bladder flushed with LRS several times, 360 ML red urine extracted from bladder. Pt sedated with 0.15 each of telazol and dexdomitor IM, reversal with 0.1 cc atipam IM. 0.27 cc buprenex SC. continue SC with buprenex BID SC, prazosin, cerenia SID, clavamox BID. recc rescue for further workup.
RE-EXAM:
cat continues to be very gentle and allows all handling and treatment without any signs of aggression
animal able to urinate, but bladder still large
red rubber sutured in place, to aid in expression of bladder
gave 150 cc lrs sq, gave 0.1 diazepam, gave 0.3 buprenex

EENT: OU clear, moderate ceruminous debris AU, No nasal d/c
Oral cavity: MM pink, CRT <2sec, Healthy dentition
PLN: WNL
H/L: No murmur or arrhythmias, Lungs clear
ABD: tense, distended abdomen
U/G: MI, very erythematous penis and peripenile area with hair clumps, able to urinate clear stream when bladder pressure applied (significant amount of pressure needed, patient crying out)
M/S: hindlimb paresis, some motor, hyperextended, reflexes intact, BCS 7/9
Neuro: mentation appropriate
A:
Middle aged MI DSH
1) Hx of partial UO, unblocked; FLUTD
2) Hindlimb paresis, nonambulatory
3) Dirty coat
P:
1) Continue cerenia, clavamox, buprenex, and prazosin as previously prescribed
2) Buprenex 0.01 mg/kg IM. Pass red rubber today to aid in urination. Give valium 0.5 mg/kg IV
3) Manually express urinary bladder BID to ensure pt is emptying
3) Keep in medial and recheck daily
4) Find rescue for further treatments/rehabilitation
