I came in the shelter as a OWNER SUR on 04/07/2015 from NY 11434, owner surrender reason stated was MOVE2PRIVA. I came in with Group/Litter #K15-009331.

04/09/2015 Exam Type RE-EXAM – Medical Rating is 2 NC – MINOR CONDITIONS NOT CONTAGIOUS, Behavior Rating is NONE, Weight 9.6 LBS.
Hx: Recheck teeth S: Quiet, calm, hides face in litterbox, tolerates exam and all handling O: BCS 7/9. BAR-H, MMs pink and moist, CRT < 2 sec EENT – Moderately dirty ears – dark brown waxy to granular discharge. No dschg OU. Heavy calculus and severe gingivitis, especially over maxillary premolars. Thin fur rostral to ears, bilaterally. H/L – NSR, NMA. Eupnic. Abd – Soft, no pain on palp, no masses palpated. M/S/I – Prefers to be sternally recumbent in cage. No skin lesions noted. UG – Female A: 1. Treated for ear mites, but ears still contain debris 2. Severe dental disease and likely resorptive lesions on max PMs P: 1. Clean ears 2. Seek veterinary care with permanent vet once placed – dental cleaning, radiographs and likely several extractions will be needed Short-term prognosis: Excellent 1088

Notes: Baisley was bought in because her previous owner moved. Baisley at the time of the assessment was displaying fearful behavior; does not seek attention from the assessor and becomes alert when the kennel was open. After a steady low approach using soft voices she relaxed a little and I was able to pet her and interact with her in a limited way. I was also able to pick her up and although she was tense no fractious behavior was display. We recommend that this cat go to a home with experienced cat parents. Reaction to assessor: does not come to front but remains neutral (2) When spoken quietly: does not come to front but remains neutral (2) Reaction to door cage opening: remains motionless (2) Reaction to touch: seems indifferent to my presence and touch (2) Try picking cat up: tolerant of this type of handling but tense.
