Medical Summary Microchip: scan negative Sex: female-Spayed Age: approximately 8 years old Mentation: BARH Eyes:eye is clear, slight clear d/c from tear ducts Ears: ear mites positive -.2 cc ivermectin sq -as per :1059 Nose: Clean Teeth:upper right canine and all molars/premolars missing, upper left canine loose (should be removed); mild staining and dental tartar BCS: 3/5 Hair/Skin: heavily matted along spine and anogenital region; clipped. Flea dirt noted, no live parasites found Nails – right front paw two distal digits grown into paw pad. Clipped, removed, and cleaned with Nolvosan Abcessed anal gland right rear. Clipped, cleaned with Nolvosan. Any injuries: seen right anal gland abscess and right front foot the last two digit injuries -checked by Vet. Behavior: friendly, allowed all handling NOSF gave- activyl-.5cc and pyrantel-1.6cc
